GLP-1 Series: Week 7 GLP-1 Medications and Your Gallbladder: What You Need to Know

Throughout our GLP-1 education series, we’ve talked about how these medications work, muscle loss, nutrition, side effects and, most recently, pancreatitis.

This week we’re turning our attention to another organ that deserves more discussion:


Your gallbladder.

Unlike some potential GLP-1 complications where research remains uncertain, there is fairly strong evidence showing an association between GLP-1 receptor agonists and an increased risk of gallbladder and biliary disease.

But there’s more to the story.

Rapid weight loss itself can increase the risk of gallstones—regardless of how the weight is lost.

So let’s look at what the medical research actually tells us.


What Does Your Gallbladder Do?

The gallbladder is a small organ located beneath your liver.

Its primary job is to store and concentrate bile, a fluid produced by your liver that helps your body digest fats.

When you eat—particularly a meal containing fat—the gallbladder contracts and releases bile into the small intestine.

Problems can develop when substances in bile harden and form gallstones.

Gallstones can sometimes exist without causing any symptoms. But when a stone blocks a bile duct, it can cause significant pain and potentially serious complications. The NIH’s National Institute of Diabetes and Digestive and Kidney Diseases explains that untreated bile-duct obstruction can lead to complications requiring medical attention.


What Does Research Show About GLP-1 Medications and Gallstones?

One of the strongest studies examining this question was a systematic review and meta-analysis published in JAMA Internal Medicine.

Researchers analyzed:

76 randomized clinical trials

involving

103,371 participants.

Compared with controls, people randomized to GLP-1 receptor agonists had a 37% higher relative risk of gallbladder or biliary disease overall.

Researchers also found increased relative risks of:

Gallstones (cholelithiasis): 27% higher

Gallbladder inflammation (cholecystitis): 36% higher

Other biliary disease: 55% higher

Importantly, these are relative increases, not the percentage of patients who developed these conditions. The absolute increase in the combined outcome was approximately 27 additional events per 10,000 patients per year in the analysis.

That’s an important distinction when talking about risk.


The Risk Appeared Greater When GLP-1s Were Used for Weight Loss

This finding is particularly relevant considering how these medications are being used today.

In the same analysis, trials specifically studying GLP-1 medications for weight loss showed a stronger association with gallbladder or biliary disease than trials involving diabetes and other conditions.

The relative risk in the weight-loss trials was approximately 2.29, compared with 1.27 in trials for diabetes or other diseases.

Researchers also found stronger associations with higher GLP-1 doses and longer treatment duration.

That doesn’t mean everyone taking a higher dose will develop gallstones.

Most won’t.

It means the increased risk is significant enough that both patients and healthcare professionals should know about it.


Why Might GLP-1 Medications Affect the Gallbladder?

There may be more than one mechanism involved.

The JAMA Internal Medicine researchers noted that GLP-1 receptor agonists may affect gallbladder motility and gallbladder emptying.

In other words, the gallbladder may not contract and empty in exactly the same way.

But there’s another major factor:

Rapid weight loss.

And this is not unique to GLP-1 medications.

The NIH’s NIDDK explains that losing weight very quickly can increase gallstone formation.

During rapid weight loss, the liver releases additional cholesterol into bile. Rapid weight loss can also interfere with normal gallbladder emptying.

So the increased gallbladder risk observed with GLP-1 treatment may involve a combination of the medication’s effects and the substantial weight loss these medications can produce.

Obesity Itself Can Also Increase Gallstone Risk

There’s another piece of context that’s easy to overlook.

Obesity itself is a risk factor for gallstones.

According to NIDDK, people with obesity may have higher cholesterol levels in their bile and may also experience changes in gallbladder function.

That means someone beginning a GLP-1 medication may already have an elevated baseline risk.

Then rapid weight loss may add another risk factor.

And GLP-1 treatment may independently influence gallbladder function.

This is why determining exactly how much risk comes from the medication versus the weight loss itself isn’t always straightforward.


What Happens When a Gallstone Causes a Blockage?

Many gallstones cause no symptoms at all.

Problems occur when a stone moves and blocks one of the ducts carrying bile.

A gallbladder attack commonly produces pain in the upper-right abdomen.

A blockage can potentially lead to:

Cholecystitis — inflammation of the gallbladder

Bile duct obstruction

Cholangitis — infection or inflammation involving the bile ducts

Pancreatitis

That last complication brings us directly back to Week 6.

Gallstones Can Cause Acute Pancreatitis

The pancreas and gallbladder share drainage anatomy near the beginning of the small intestine.

A gallstone that becomes lodged near this area can interfere with pancreatic drainage and trigger inflammation of the pancreas.

This is called gallstone pancreatitis.

So when discussing pancreatitis in someone taking a GLP-1 medication, there can potentially be more than one factor involved.

The medication may be under consideration.

Gallstones may be involved.

Rapid weight loss may contribute to gallstone formation.

And the individual may have had gallbladder risk factors before ever starting treatment.

Medicine is rarely as simple as identifying one factor and assuming it explains everything.


What Symptoms Should You Pay Attention To?

If you’re taking a GLP-1 medication, don’t automatically dismiss significant abdominal symptoms as normal medication-related nausea.

Symptoms of a gallbladder attack commonly include pain in the upper-right abdomen, while gallstone complications can produce more significant symptoms.

Seek medical advice for concerning symptoms such as persistent or severe upper abdominal pain, fever, vomiting, jaundice or yellowing of the skin or eyes.

Severe abdominal pain—particularly when accompanied by fever, vomiting or jaundice—requires prompt medical evaluation.


Can You Reduce Your Gallstone Risk?

There is no strategy that can guarantee someone won’t develop gallstones.

However, NIDDK emphasizes avoiding extremely rapid weight loss when possible. It also recommends a healthy eating pattern emphasizing fiber-rich foods such as vegetables, fruits, beans and whole grains while limiting refined carbohydrates and unhealthy fats.

This reinforces something we’ve discussed throughout this series:


The goal shouldn’t simply be losing as much weight as possible as quickly as possible.

How you lose weight matters.

Your nutrition matters.

Your muscle matters.

And your overall health matters.

Put the Risk Into Perspective

It’s important that we don’t turn an increased risk into an exaggerated claim.

The research does not tell us:

“GLP-1 medications will damage your gallbladder.”

It tells us something more specific:

People randomized to GLP-1 receptor agonists in clinical trials experienced gallbladder and biliary disease more frequently than people in the comparison groups.

The absolute increase was considerably smaller than the relative percentages can make it sound.

Both pieces of information matter.

That’s what informed healthcare decisions should look like—understanding benefits, relative risks, absolute risks and your own individual health history.


The Bottom Line

GLP-1 medications have been associated with an increased risk of gallstones, gallbladder inflammation and other biliary diseases in randomized clinical trials.

The association appears stronger when these medications are used for weight loss and at higher doses or for longer durations.

At the same time, rapid weight loss itself is a recognized risk factor for gallstone formation.

So this isn’t about frightening anyone away from medication.

And it isn’t about ignoring a documented risk because the medication may provide benefits.

It’s about knowing both.

Know your medication.

Know your individual risk factors.

Know which symptoms deserve medical attention.

And don’t be afraid to ask your healthcare provider questions.
